Annual Registration Forms:

• Online Registration for existing students through JMC Parent Portal - all forms will be electronically signed through online registration.

New Student Forms:

• Admissions Forms:

  • Registration Forms on paper in Welcome Packet
  • Proof of Address (Bank statement, item from your mail, driver's license)
  • Parent Information/Emergency Contact Information 
  • Parental Pledge Signed by Parent(s)/Guardian(s) of student(s)
  • Proof of Birth Date (Birth certificate, baptism record, immunization record)

• Health Forms:

  • Physical Examination completed by family physician
  • Current Immunizations
  • Vision Screening (Vision screenings are also done here at school in the fall for all students)
  • Dental Screening

  • Physical Examination completed by family physician

• 3rd Grade Student Required Forms:

• 7th Grade Student Required Forms:

  • Updated Immunizations

Athletics (5th - 8th Grade students participating in any athletics throughout the school year):

• Concussion Form - Required for any athlete each school year (signed electronically on JMC at registration for existing students)
• Athletic Physical Form - Required annually for any athlete to be completed from a physician

Other Health Forms:

• Diet Modification Request Form - This is for a food allergy or milk intolerance
• Athorization for Medication Administration - This is required if a medication is to be administered during the school day

• Class Supply List - 25/26 School year
• Free/Reduced Lunch Application - 24/25 application 
• Public Release for schools operating the National School Lunch and Breakfast Program
• NICSTO Eligibility Application
• NICSTO Contribution Memo
• Authorization for Direct Credit (ACH) for Tuition and/or Lunch Account
